Resolved Question:

hi there
I just got my blood test results.
Cholestrol is 6.5
LDL 3.6
HDL 2.3
VLDL 0.6
Triglyceride 1.20

While the overall score is deemed 'high' is there any real risk? Should I change something in my diet - I exercise and don't smoke. do love my red wine. Have stopped eating cheese having had a very high 7.7 score 6 months ago.

Dr. Shahzad MD :

Hi there. Thank you for your question. I will be helping you out with your queries

Dr. Shahzad MD :

welcome

JACUSTOMER-xb16245v- :

Thank you

Dr. Shahzad MD :

do you have any other medical problems?

Dr. Shahzad MD :

Are you on any medications for anything?

JACUSTOMER-xb16245v- :

yes I take 150mcg of Eltroxin daily mon to fri but just 100 on Sat and Sun..I had a subtotal thyroidectomy when I was 18..just one tenth left. interestingly i have to now reduce to 125 according to the results also of the tests I took last week. I used to eat cheese and gave up, was vegetarian and now eat fish if sustainably caught..so this is a change in my diet since January

JACUSTOMER-xb16245v- :

kind of wonder if the change in diet also makes me not need so much eltroxin..i didn't used to eat fish before ever. but that's neither here nor there I guess in relation to cholestrol?

Dr. Shahzad MD :

since how long are you on eltroxin/

JACUSTOMER-xb16245v- :

28 years i guess

Dr. Shahzad MD :

ok

Dr. Shahzad MD :

Did you doctor suggest any medication for your high cholesterol or just diet therapy or nothing yet?

JACUSTOMER-xb16245v- :

didn't suggest medication, i just took the advice to reduce/cut out cheese.

Dr. Shahzad MD :

There is a higher risk of having heart disease and stroke with your score yes but its not something really alarming.

Dr. Shahzad MD :

It can be controlled.

Dr. Shahzad MD :

The first thing that we always try is diet and exercise.

JACUSTOMER-xb16245v- :

naturally?

Dr. Shahzad MD :

Yes you can do that by increasing soluble fiber in your diet.

Dr. Shahzad MD :

At an intake of 8 grams per day, soluble fiber lowers total cholesterol and lowers the LDL:HDL cholesterol ratio.

Dr. Shahzad MD :

Common sources include oats, barley, legumes, and many fruits and vegetables.

JACUSTOMER-xb16245v- :

ok. not sure what I can improve with my diet. It is almost ridiculously healthy. totally organic and very high in fiber. veg, tofu, fish fruit beans nuts and I exercise a lot (3 dogs and I love cycling)

Dr. Shahzad MD :

You should keep on doing what you are doing.

Dr. Shahzad MD :

Repeat the test in three to six months time.

Dr. Shahzad MD :

If its still high then you will need a low dose of a cholesterol medication (statin).

JACUSTOMER-xb16245v- :

but how is the ratio worked out and what is desirable? How do I 'score' ratio wise?

Dr. Shahzad MD :

Actually your total cholesterol is high.

Dr. Shahzad MD :

and so is your LDL

Dr. Shahzad MD :

Which makes your score high.

JACUSTOMER-xb16245v- :

but this statin option is surely for cases of very high LDL and low HDL? I am always confused...

Dr. Shahzad MD :

Statins are for high LDL I agree.

Dr. Shahzad MD :

and usually when the LDL is high the HDL is also low

Dr. Shahzad MD :

But not in your case.

Dr. Shahzad MD :

In your case the cholesterol is high and the LDL is high whereas the HDL is fine.

Dr. Shahzad MD :

triglyceride is also fine.

Dr. Shahzad MD :

But again I would just like that you should keep on doing what you are doing at the present moment.

JACUSTOMER-xb16245v- :

so if I had a low HDL and the overall score was 'within the limit' but the LDL was still the same..no statins or any other medication would be advised?

Dr. Shahzad MD :

And repeat the test after a period of three to six months.

Dr. Shahzad MD :

The score would have been higher.

Dr. Shahzad MD :

The higher the HDL (good cholesterol) the lower the score.

JACUSTOMER-xb16245v- :

ok thanks I'll work on that so. But new theories are coming up every week aren't they..hard to know what is best - the 'score' ideas seems rather simplistic without looking at the whole picture. I would be very reluctant to take statins..I think my HDL is trying to help clear my arteries!! Anyway, thanks again.

Dr. Shahzad MD :

Here is how the score is calculated:

Dr. Shahzad MD :

http://cvdrisk.nhlbi.nih.gov/

Dr. Shahzad MD :

units for cholesterol are different here.

JACUSTOMER-xb16245v- :

ok...I guess every individual is different and what would my units translate to in the other measures? Is there somewhere I can find out?

JACUSTOMER-xb16245v- :

Could red wine (cabernet sauvignon) be a factor?

Dr. Shahzad MD :

You multiply your number with 38.61

Dr. Shahzad MD :

it will be converted into milligrams

JACUSTOMER-xb16245v- :

thanks

Dr. Shahzad MD :

If you are taking red wine in moderation then its not a factor.

Dr. Shahzad MD :

Anymore questions?

JACUSTOMER-xb16245v- :

ok..often I have more than one or two glasses that's too much? I know the advice is one a day..but realistically this surely is not so bad to have 2?

Dr. Shahzad MD :

Try to limit it as much as you can but one or two glasses is not too much.

JACUSTOMER-xb16245v- :

may i ask one more question in relation to increasing fiber, I've added more fruits to my diet, and this week have a kind of strange sensation, almost like an itch in the lower left area of my tummy, above my pubic bone but more towards my left hip..quite deep internally..could this be connected..?

Dr. Shahzad MD :

No

Dr. Shahzad MD :

I do not think this is connected

Dr. Shahzad MD :

I am not sure what this sensation is but I am sure its not connected.

Dr. Shahzad MD :

Anymore questions?

JACUSTOMER-xb16245v- :

ok thanks that's fine. Bye

JACUSTOMER-xb16245v- :

i thought it may be trapped wind...or overloading my intestines..thanks anyway I shall leave now.
