My very close friend has an adopted child of ten years old.

My very close friend has an adopted child of ten years old. My friend is in her fifties, not in the best of health. She and I wish in the event of her death, for me to take responsibility for the child. How can we formalise this in writing ?

Yes she is a single parent and addopted the child alone. The child was fostered then addopted.
