From the description you've given this will be due to a dual mass flywheel failure. Usually if the flywheel fails you find either the noise changes as you press the clutch or there is a vibration through the clutch pedal.
Unfortunately if the flywheel has failed then the transmission needs removing to access the clutch and flywheel.
