Hello, I hope the following assists you.

From the description you've given this will be due to a dual mass flywheel failure. Usually if the flywheel fails you find either the noise changes as you press the clutch or there is a vibration through the clutch pedal.

Unfortunately if the flywheel has failed then the transmission needs removing to access the clutch and flywheel. By far the cheapest option and to prevent repeat failures is to replace the dual mass flywheel with a solid flywheel conversion such as http://www.ebay.co.uk/itm/AC-DELCO-SOLID-FLYWHEEL-CONVERSION-KIT-AC925K-/301156457607?pt=UK_CarsParts_Vehicles_CarParts_SM&fits=Model%3AMondeo&hash=item461e52dc87 a new dual mass flywheel is around £300 from Ford without the clutch so you can see the difference when compared to the solid flywheel conversion.
