I hereby apply for ILR on the basis of 10 years long residence.

Give details of your 10 years stay in the UK, what visas you held and the dates of these visas.

Explain gaps in your continous stay e.g. you were refused leave to remain on xxx which you appealed/re-applied and were successful.

List all your documents supporting the application and SET(O) form.

I hereby request you to grant me ILR on the above basis and where necessary, exercise discretion in my favour due to >>>>>

In continuation of our discussion, reading through my SAR summary, I can see that my new application was made in January 2011and the Appeal Rights Exhausted date was towards the end of Feb 2011.

 

Does this indicates no gap in my 10yrs long stay?

A new application during an appeal period is not covered by S. 3C.

So there is a gap which may not be held against you.
