hi the child is 13 years old..There is no residence order.The father was ordered outdue violent behaviour 4 years ago.he has never been seen or heard from

according to the polish consulate only the father can sign the passport forms.The whereabout of the father is not known. He has failed to pay child maintaince for the past 4 years.

YOu will have to check with your embassy but I understand that if you have a Residence Order stating that the child is in your care then you will not require his signature.
You will need to take active steps to try and find him first - by using an enquiry agent - but you can then apply to the Court for a Residence Order which should solve the problem
The application is made on a form C100 and the fee is £200
