I have a pension provider in Switzerland and they would like proof that I am not married to my long term girlfriend before they remit the money back to my Uk bank account

A statutory declaration is a legal document defined under the law. It is similar to a statement made under oath however, it is not sworn.
Statutory declarations are commonly used to allow a person to affirm something to be true for the purposes of satisfying some legal requirement or regulation when no other evidence is available. You can use it as a way of declaring that you and your girlfiend are not married.

You need to be aware that stating something false in a statutory declaration can lead to arrest and imprisonment.
I A.B. do solemnly and sincerely declare, that and I make this solemn declaration conscientiously believing the same to be true, and by virtue of the provisions of an Act made and passed in the year of the reign of his present Majesty, intituled “An Act” (here insert the title of this Act).
This is the form of the declaration set out in the schedule to the Statutory DEclarations Act 1835. You will need to draft a document that does the following:
- intially sets out your full name, date of birth and address.
- contains the fact that you are declaring (ie that you are not married),
does it need to be notarized?
You will need to take the document to a solicitor or notary public and make the declaration on front of them and pay a small fee (£5.00 I think).

I have set out above a format you could use for your statutory declaration and then take it to a solicitor for declaring.
