I have to give a Section 21 Notice to a tenant. The one I have

I have to give a Section 21 Notice to a tenant. The one I have down loaded from the internet is strait forward and simple but finishes with Notes about the way in which the Section 21 can be issued and finishes with information regarding how the tenant can get legal advice etcetera.
Do I have to include the "Notes" in the document or can I just keep it simple?
I don't want to arm my tenant with this information.
