I have a county court Order of possession under ground 8 that

Resolved Question:

-I have a county court Order of possession under ground 8 that my tenant should give up my property by 29 May 2015
-The defendant pay me £5,878.27 for rent arrears and £46.66 per day from 03 May 2015 until possession given to me.
Questions
1. On form N325 para 4 (A) What should I write for 'Balance Due at the date of request?
2(B) What should I write for 'Amount for which warrant to issue?
Issue Fee I understand is £110
Also above the signature field on right column 'I certify that..
(1) should stay as it is?
(2) Should be struck out?

Many thanks Alex, I appreciate this.
-Today council paid me rent for the month of May which is £350.03 less than the actual rent.
-Tenant should pay me on 3rd of every month
- I am not very clear about 4(A) and 4(B)
-4(A) Balance due at the date of this request ( Is this todays date?)
-4(B) Amount for which warrant to issue ( What is it? Should this be same as 4(A)?)

Please clarify followings if I fill up the form today i.e 30/05/2015. Should it be like this?

4(A) £6228.27 (5878.27 + 350.03)
4(B) £6228.27
Issue fee £ 120
Total £6348.27
