I have persistent swelling under my arm and breast pain that extends to my back and collar bone, i have spoken to my GP who gave me painkillers but my arm is now swollen, should I be concerned

I haven't had a mammogram as I've just turned 50 this month

None that I'm aware of. The NHS here doesn't allow you to request one I will be offered one at some point now because of my age

actually with swelling under the arm and pain in the breast I would go back to your GP.
Insist on an explanation, or a mammogram.

It should be possible to exclude a noticeable cancer on exam, but not in every case...so mammogram is currently indicated.

if this indicates that something may be sinister would the pain be constant or intermittent, my arm is uncomfortable rather than painful but the pain in my back and collar bone comes and goes without due reason. I am sure it is not muscular?

It could be either.

This could all be muscular...but you are due the mammogram.
