Dr. David :

This is Dr. David

Dr. David :

this could be due to tinnitus

Dr. David :

or it could be due to inflammation of the TMJ joint

Dr. David :

which is the jaw joint which moves when you chew.

Dr. David :

you need to see a ENT doctor and get your ears checked for fluid behind the ear drum

Dr. David :

and also have someone check your TMJ joint for inflammation.

Dr. David :

do you grind your teeth at night? this can cause excessive inflammation of your TMJ joint.

Dr. David :

if this is the case, you might benefit from a bite guard.

Customer:

Hi doctor yes I have fluid behind my eardrum and have been given nasonex for it and it hasnt worked also took antibiotics and they have worked either are there any other treatments

Dr. David :

yes, you should see an ENT doctor

Dr. David :

you might need to have tubes put in your ear drums to help drain out the fluid.

Dr. David :

you can also try to open up your eustachian tube by yawning and thrusting your jaw forward.

Dr. David :

some people can do this voluntarily. but it can take practice.

Dr. David :

the eustachian tube is the tube which leads from your inner ear to your throat and helps drain fluid out of your inner ear.

Customer:

Yeah I have sinus issues also but is it mormal to fluid to remain so long?

Dr. David :

no, it is not normal.

Dr. David :

that is why I think you need to try to see an ENT doctor

Dr. David :

or ears nose throat otolaryngologist expert.

Dr. David :

and see what else you can do to help get rid of that fluid.

Dr. David :

you might need allergy medications

Customer:

Could there be a sinister cause?

Dr. David :

most likely not.

Dr. David :

most likely the fluid is just not draining.

Dr. David :

and most likely the fluid is not infected if you have been on antibiotics.

Dr. David :

so the fluid is most likely sterile clean fluid.

Dr. David :

it is just bothering your inner ear.

Dr. David :

have you tried to yawn and thrust your lower jaw forward to try to open up your eustachian tube?

Dr. David :

it can take practice.

Customer:

It is a nuisance constant pulsing sensation is so frustrating could it be possible to get this after plane journey

Dr. David :

yes, after pressure changes on a plane this happens frequently.

Customer:

Havent tried that techniques yet

Dr. David :

best of luck to you.

Dr. David :

I can understand your frustration.

Dr. David :

let me know if you have other questions.

Customer:

Plane journey was last september though?

Dr. David :

well, then that is a long time for fluid to be there.

Dr. David :

also during the winter, when there are more infections, it is common to get an inner ear infection from a virus or bacteria

Dr. David :

which can take a while for the fluid to resolve.

Customer:

My doc also offered me cipralex due to the anxiety it is causing...silly question but is it ok to have wine with this med with dinner

Dr. David :

most likely it should be ok to have wine.

Dr. David :

let your doctor know as well that you are drinking wine with dinner.

Dr. David :

I hope the cipralex helps with your anxiety.

Dr. David :

it can take 4-8 weeks to start working for you.

Dr. David :

so don't stop the cipralex if it doesn't work.

Dr. David :

best of luck to you.

Customer:

O only mean one or two glasses no more really.....you ment

Dr. David :

what did you mean?

Customer:

Usually only have one or two glasses

Customer:

Im always unsure of meds

Customer:

Dont lime taking them unless its necessary

Dr. David :

well, it sounds like in this situation, your doctor thinks you have excessive anxiety

Dr. David :

and so cipralex can help decrease anxiety.

Dr. David :

best of luck to you

Dr. David :

let me know if you have other questions.

Customer:

Yes I do have anxiety issues which have been brought on by my ear problem hopefully as u say a ENT wil sort it out.

Dr. David :

yes.

Dr. David :

hopefully the cipralex will help your anxiety as well.

Dr. David :

are you not taking the cipralex?

Customer:

Im afraid to start the med to be honest

Dr. David :

it sounds like you are letting your anxiety keep you from taking it.

Customer:

Yes doctor thats wat it is

Dr. David :

well, until you start some treatment for your excessive anxiety, you will continue to battle with anxiety .

Dr. David :

in your prior questions you were worried about brain tumors causing your symptoms.

Dr. David :

this is no way to live.

Customer:

Its the side effects that are putting me off

Dr. David :

you need to get some therapy to help your anxiety

Dr. David :

and also to talk to a psychotherapy counselor regularly as well

Dr. David :

did your doctor set you up to talk to a counselor regularly?

Customer:

No its embarrassing feeling like this I dont mention my problems to nobody not even my partner

Dr. David :

well, you will not get better if you don't get treatment for your anxiety

Dr. David :

and if you don't talk to anyone about your symptoms.

Customer:

I feel its a sign of weakness I am law student and dont want to mess up my studies taking meds

Dr. David :

it is a sign of weakness not take help when you need it.

Dr. David :

it is a sign of weakness not to take help when you need it.

Customer:

Yes I know......

Dr. David :

let me know if you have other questions.

Customer:

Ok just give me your final piece of advice regarding anxiety and also the ear problem...is cipralex a safe med

Customer:

Thats all my questions

Dr. David :

yes, cipralex is a safe medication

Dr. David :

and is used often for patients who have excessive anxiety or depression.

Dr. David :

it is known as lexapro in the US

Dr. David :

and is used here as well.

Dr. David :

it can take 4-8 weeks to start working.

Customer:

Customer: replied 2 years ago.

Hi doctor just been to ent..no fluid in ear hearing is perfect and no swelling of nasal lining...had no explanation for the wooshing only not to focus on it.....

Expert:  Dr. David replied 2 years ago.
do you have a history of excessive anxiety? that could be causing you to hear this wooshing noise.

did the ENT doctor check for TMJ inflammation of the jaw? are you biting down frequently or grinding your teeth at night?
Customer: replied 2 years ago.

Yeah he checked for thst too and was fine...I prob hold alot of tension in jaw..never noticed if i grind my teeth much...sounds like it could be anxiety disorder. .though how can this causr wooshing?

Expert:  Dr. David replied 2 years ago.
many people with anxiety do experience tinnitus or ringing or whoosing sounds in the ear.

anxiety activates the fight or flight system, and this places a great deal of pressure on the nerves, blood flow, body heat, and more

it is very likely that this pressure and stress travels up into your inner ear, and leads to the experience of tinnitus

you need to talk to your doctor about treatment for your anxiety symptoms. treatment for your excessive anxiety could help decrease this whoosing sound.
Customer: replied 2 years ago.

Ok thanks I have started cipralex 5 mg and have seen no sideveffwcts so far thankfully hope I dont have any

Expert:  Dr. David replied 2 years ago.
that is good you have started cipralex.

that is a low starting dose of cipralex.

remember, it can take 4-8 weeks for the cipralex to start working.

after that if it doesn't work, talk to your doctor about increasing the dose up to 10mg

20 mg a day is the max dose of cipralex.

good luck to you.
Customer: replied 2 years ago.

If the 5mg works for me I would be happy enough to stay on that....the less the better I think.....

Expert:  Dr. David replied 2 years ago.
yes, I hope the 5mg works for your anxiety symptoms

good luck .
Customer: replied 2 years ago.

Anxiety is horrible..it makes ye feel like absolute rubbish

Expert:  Dr. David replied 2 years ago.
sounds like it.

I hope the 5mg works for you.

you have to give it 4-8 weeks to work.

Customer: replied 2 years ago.

Expert:  Dr. David replied 2 years ago.

Customer: replied 2 years ago.

Wat are most common side effects of cipralex

Expert:  Dr. David replied 2 years ago.
nausea, delayed ejaculation, diarrhea, sleepiness.

but try not to stress about these side effects.

if you worry too much about them, your anxiety will make you think you are having them.
Customer: replied 2 years ago.

Ok thanks I know that side effects usually dont last long and I askwd pharmacist about wine and he said its fine because 5 mg is a low dose. .thanks for all your info
