HI, I am a 43 year old female who has been suffering from problems

Resolved Question:

HI, I am a 43 year old female who has been suffering from problems with her throat since late last summer. There are swollen glands in my neck and I lost my voice about 5 weeks ago. My doctor sent me to ENT and the consultant did an endoscopy and told me that I had had an infection which was now gone, and that the muscles round my vocal cords were tight - hence the no voice. He suggested I go back to see him in three months and if I still had no voice he would arrange for a speech therapist to see me.
Surely if it was an infection which is gone the glands in my neck would have gone down by now. I have not had any symptoms of infection since a sinus infection last july. Finally, when I was young I was an operatically trained singer. While I haven't sung in 20 years, I remember how to relax the vocal cords and do not strain or over use my voice in any way. I'm wondering if they have missed something. Can you help?

I was sent for an ultrasound back in early January to be told that they were normal - but my GP has said that he can feel them raised by physical examination since then.

There are several issues to consider in this situation.

From the perspective of the throat, the endoscopy done by the ENT physician is the most sensitive test to assure that there is no disease in the throat. So, if anything has been missed, it would be from the perspective of the swollen lymph nodes.

From the perspective of the lymph nodes, I am obviously at a disadvantage since I cannot examine the lymph nodes, but I can address the usual approach to persistently swollen lymph nodes. If the lymph nodes do not feel worrisome and appear normal on an ultrasound, most doctors will typically just follow the lymph nodes. However, if the lymph nodes are getting more prominent over time, then it would be appropriate to consider a biopsy of the lymph nodes. If your doctor feels that there are more prominent (if that is what he means by feeling them raised), then it would be appropriate to consult with a surgeon about a possible biopsy. If the physical examination is uncertain, then it would be appropriate to repeat the ultrasound and see if the lymph nodes are larger.
