My daughter tells me she feels she has a small lump in her

Resolved Question:

My daughter tells me she feels she has a small lump in her throat ( like a sweet stuck).
She has felt this for ten days. Her throat is occasionally sore. Her appetite is not affected.
Her GP has suggested she leaves it for another ten days then, if she still has the condition she will be referred to a specialist.
My daughter has three small children, the youngest four, her husband is currently divorcing her and works away from home. I wonder if it would be wise to get her referred to a specialist sooner rather than later?

Ken :

Hello,

Ken :

I am Dr Ken and I will be helping you today.

Ken :

Does she have any associated symptoms like -

Ken :

Fever/ cough/ pain in abdomen/ reflux disease?

Ken :

Does she have any diagnosed medical condition?

Customer:

Some acid reflux. No other condition

Ken :

I am happy that she does not have any other symptoms.

Ken :

Let me explain a bit.

Ken :

Feeling of lump in throat indicates -

Ken :

Irritation of the mucosa {inner lining of the throat}.

Ken :

The cause can be varied -

Ken :

Inflammation/ Infection {viral or bacterial}/ allergy/ post-nasal drip/ reflux disease.

Ken :

Most common cause is viral infection. This generally does not need any antibiotics and individual gets well in a week.

Ken :

In her case -

Ken :

I would suspect reflux as my first though because -

Ken :

She is going through emotionally disturbed phase and this leads to increased acid secretion in the stomach - causing more reflux -

Ken :

Irritating the mucosa of throat and so the symptoms.

Ken :

However

Ken :

Other causes of increased acid secretion like - H.pylorie infection also needs to be ruled out.

Ken :

Next step -

Ken :

A physical evaluation along with few investigations like throat swab {to look for any infection}/ blood tests and upper GI scopy to get more answers.

Ken :

Till than -

Ken :

Steam inhalation/ Warm water gargles {with pinch of salt}/ Benadryl, Ibuprofen and TUMS should give relief.

Ken :

She needs to have balanced diet, adequate fluid/ avoid alcohol, smoking or spicy food/ have multiple small meals instead of few large/ raise the head part of her bed.

Ken :

Stay away from anxiety {Yoga, meditation help}.

Ken :

Please do not worry, she will be fine.

Ken :

Does this make sense?

Customer:

Thank you. My daughter does smoke occasionally but otherwise she eats and excellent diet and does indeed do yoga. I wonder what your second thought woiud

Ken :

Second thought would be viral infection.

Ken :

In abscence of fever/ weight loss or loss of appetite -

Ken :

I would not think of bacterial infection or Cancer {I think you are worried of this}.

Customer:

Am pretty sure she doesn't have and infection, no fever, good appetite etc

Ken :

You are right. Hence I thought of reflux.

Customer:

Yes, this is a concern. Her husband had colon cancer two years ago and her sister in law died of secondaries from melanoma in 2011

Ken :

Please do not panic. Her symptom does not make me think of Cancer.

Customer:

OK. Thanks very much you have reassured and anxious old woman a bit

Ken :

Stay calm. Wish her well.

Ken :

Best

Ken :

:)
