I have a 7 week (approx) pregnancy with twins. At the 4th week

I have a 7 week (approx) pregnancy with twins. At the 4th week I was rushed into hospital with bleeding and cramps. A first Doppler scan took place which showed an hematoma and the 2 sacks with feeding sacks (Eggs with yolks) properly attached to the uterus. I was not prescribed any medicine or drug. I went home and extremely went into rest mood. I lie down most of the time for the rest of the pregnancy, taking folic acid and pregnancy vitamins. I only stand up to eat and go to the toilet. I continue with light bleeding and cloths for at least 7 days.
At the 6th week, after not bleeding for at least 4 days, the hematoma showed some healing and less blood was found in the uterus. Two embryos in their sacks were shown, but concern was raised as placenta shape was not round (like a balloon) and embryos didn't show a heart beat on the scan. Doctor delivered bad news about the pregnancy, but did consider the point of embryos showing in sacks, when in previous scan they were not showing. No pain or cramps were occurring at this point, no medicine was given and I continue to eat and rest. Pregnancy symptoms increased such as nausea and headache.
At 7th week embryos didn't show any growth and heart beat was still not there. Doctors measure there was not blood irrigation showing to the uterus. Measure of one embryo corresponded to 4th week of growth and measure of second embryo corresponded to 2nd week of growth. This seems to be the apparent period when embryos stopped growing. There hasn't been cramps or pain or any bleeding.
I was sent to the miscarriage unit, but I got the following questions:
a) Why did the babies stop growing?
b) Is it possible that if I give them more time, eg: 2 more weeks they will miraculously grow or show a heart beat?
c) Why my body hasn't performed the miscarriage naturally? Is it all the rest I am taking the reason?
d) Do you recommend a surgical procedure to perform the miscarriage? They are recommending the vacuum suction procedure, is this safe? Are there any side effects to it?
I really value your opinion, I am desperate and really need to have piece of mind before I take a final decision.

They only perform scans and from that they are convinced the pregnancy is not on-going

usually they will want to do a D&C (surgical procedure after 9 weeks)

Yes, I have seen some babies miraculously grow

well, if they had a heartbeat and no longer have one, then you don't have any luck

but if they never had one, then it is possible that you may see one on a repeat scan

yes, because the measurements are not 100% accurate- they are a best estimation

Dr. Natasha :

and how the babies are seen on the scan

so you have two options

1. quant bhcg- see if it is rising

2. repeat scan in 1 week

Honestly, I would wait it out and get either 1 or 2

I would not terminate quite yet

I would want conclusive evidence

Is the quant bhcg accurate? They said my body will still have high levels of hormones due to twin pregnancy

the bhcg is a baseline- they measure how quickly it is rising and that is static (at least 2x every 48 hours)

What if the bhcg is rising? What happens then?

Then you know the pregnancy is failing :-/

it is possible that they did not get the measurements correctly and that they really are growing and you need to check again

It went well, I supposed...

I had the D&C procedure after confirming that I definitely had a retained misscarriage.

I had bledding for almost 2 weeks and now I am doing fine.
