My wife is suffering with severe breathing problems suffers

My wife is suffering with severe breathing problems suffers with copd she stopped smoking 6 months ago but systems are getting worse she is under what they call the rocket team to do exercises to help her breathing but she cannot do them -she barely eats from day to day what do you suggest please

oxygen 92 but drops to 80 when any form of exertion is done she has not eaten for weeks as it makes her vomit she is only on inhalors which do not seem to help

There are several issues to consider in this situation.

First, there are several different types of inhalers that may help with COPD, although there is no inhaler that works for 100% of patients. A blue inhaler is typically a short-acting bronchodilator, also called a β-agonist. Other inhalers that can be considered would be a long-acting β-agonist, a steroid inhaler, and an anticholinergic drug. Some of these medicines are also available in combination inhalers that contain two complementary types of medicines.

For someone that is having more significant symptoms despite a blue inhaler, I would typically add both a steroid inhaler and an anticholinergic inhaler.

It also would be appropriate for her to be considered for oxygen replacement. Her baseline level of 92 is not severe enough to warrant oxygen supplementation, but the level dropping to 80 with exertion is worrisome and would be appropriate for oxygen supplementation. When considering oxygen supplementation, it is not as simple as adjusting the oxygen level to achieve a desirable oxygen level, as oxygen supplementation can also increase carbon dioxide levels, which can get to dangerous levels, so there should be testing to assure that just enough oxygen is provided without having too much of an increase in carbon dioxide.

She would need to discuss these other inhalers and oxygen supplementation with her physician.

As for the stomach and vomiting with eating, this can occur in someone with severe COPD, and can be related to acid irritation in the stomach. There are other possible causes, but there are several over the counter medicines that can be used to ease acid levels that may be helpful, including Zantac, Prilosec, or Prevacid.
