Onset pain approx 3 months ago lower right ribcage area. Pulsating

Customer Question

Onset pain approx 3 months ago lower right ribcage area. Pulsating and intermittent. Is now contestant spreading to other side and radiating up ribcage wall and back under armpits into upper arms. Pulsating and stabbing sensation over entire area
Over this period bowel movements have gone from daily to constipation unless daily 2-3 dosages of laxido just to be able to go. Recently have frequent need (5-6 times n hour) to urinate and must go wen sensation to go comes.
Most recent new event to symptoms was waking up one morning with searing burning pain at top of left leg the level of pain caused an involuntary eviction of bowel. Not faecal matter but a substantial amount of foul smelling ( bleachy) mucus. I have been left with a numbness and occasional stinging sensation at to of left leg and into inside top of right leg and feet which always feel cold.
Numerous trips to GP full blood tests which all come back with nothing abnormal found and within normal parameters. Two trips to A&E after passing out with pain. Head CT scans clear. Full blood checks clear.
Chest X-rays clear.
Last A&E visit 24 hours ago advised I had an anxiety attack even tho I gave description as above
Previous history. Breast cancer with LD Flap reconstruction seven years ago
Only other poss related incident around same time as pain onset was fall getting out of bath but was inner left arm bruising . Chest X-ray showed no rib damage.
I am currently at level 8 pain after pain relief of co-codemol and naproxen - max daily 24 dosage

Thanks for this questionI'm Dr. Phil and I'm ready to helpThis sounds like extreme irritable bowel syndrome.Has this been considered?
Customer: replied 10 months ago.
Also on medical history. Degenerative c5-c6 disc and disc spacing upper n lower discs to this
Expert:  Dr. Phil, MD replied 10 months ago.
I think the discs are causing some of the nerve issues but the main belly pain issues is extreme irritable bowel

Ruled out by both hospital n GP . No other IBS symptoms or markers in blood tests. Pain started in lower right ribcage radiating upwards and no lower abdominal pain or tenderness.
Expert:  Dr. Phil, MD replied 10 months ago.
There are no blood markersIBS is a diagnosis of exclusionMeaning ruling out other things I have treated dozens of cases of IBS.I'm quite sure it isunless it is celiac disease. have you been tested for that
Customer: replied 10 months ago.
markers show no inflammation. No bloating no flatulance, diarrhoea tho have lost about 5 kilos weight in past fortnight but think more stress related to this. My daughter has had IBS since she was a teenager and tho not medically qualified we have also thoroughly compared symptoms she has experienced during this time and mine are not atypical by her experience as a sufferer
Expert:  Dr. Phil, MD replied 10 months ago.
IBS can vary widely amond individualsThere are many types of IBSconstipation predominant, diarrhea predominant and mixedTrust me on this. I have seen many many case of IBS and that is what this is
Customer: replied 10 months ago.
I will contact my GP in morning to make appointment to discuss if u can advise of questions I need to b asking, tests to b carried out, methods of exclusions need to be done to verify/ rule out IBS or Celiac

thank you very much. In the meantime, try the fodmap diet. https://stanfordhealthcare.org/content/dam/SHC/for-patients-component/programs-services/clinical-nutrition-services/docs/pdf-lowfodmapdiet.pdfplease don't forget a positive rating. I appreciate it

Please can u review my last response and advise. My own daily diet is very similar to this as a life style choice with the exception of being dairy free and I don't drink either tea or coffee
Expert:  Dr. Phil, MD replied 10 months ago.
The best test is going to be a blood test for celiac antibodiesjust request that
Customer: replied 10 months ago.
Is that the 'gluten tolerance' (layman a terms) blood test. If it is had that done just before Xmas n all clear
Expert:  Dr. Phil, MD replied 10 months ago.
That's good. That rules out celiac thenIt has to be IBS
Customer: replied 10 months ago.
Given this has been going on for so long and GP already ruling it out. What specifically should I be asking him to do for me now to get a diagnosis to confirm or not as case mayb without being fobbed of by him. At least to show I'm somewhat informed
Expert:  Dr. Phil, MD replied 10 months ago.
You would have to do what is called a therapeutic trial at this pointsince there is no diagnostic test for IBS, try a medicine like bentyl and see if it helpsif it does, you have your diagnosis

Please don't forget a positive rating.
I appreciate it
