This is Dr. David
I am sorry your father has had a stroke.
it sounds like he is very much prone to developing blood clots and sending them to his brain.
he needs to get an ultrasound scan of the blood vessels of his neck and heart to check for plaques which can dislodge and spread to the brain.
he may need to go on stronger blood thinners than just aspirin.
he should get a brain MRI scan which can look at his brain with better detail
he should undergo some rehabilitation therapy after his strokes to help him recover.
these might be difficult for him depending on where in Africa he is

2. Lisinopril tablets BP 10mg (1 in the morning)
3. Simvastatin 20mg film-coated tablets (1 at night)

5. Soluble asprine (Dissolve 1 in half glass a water a day). What do you think. Any advice will be much appreciated thank you

it sounds like he is taking medications for high blood pressure (lisinopril) and for diabetes (metformin) and high cholesterol (simvastatin) and for heart blood vessel spasms or chest pain (nifecard)
it sounds like he has a lot of risks factors which can cause him to have stroke.
he might benefit from going on a vegan diet to help lower his cholesterol levels and help him reverse diabetes and help him loose weight to help his high blood pressure.

MRI scan is needed for the brain
an ultrasound of of the neck arteries and heart is needed to check for plaques which can spread to the brain and cause strokes.
http://www.ncbi.nlm.nih.gov/pmc/articles/PMC4031793/
is a good article for you to read
usually coumadin or warfarin is used to thin the blood to prevent blood clots from forming and spreading to the brain.
but his level of coumadin or warfarin has to be monitored by a doctor

Hello Doctor David,
It seems no doctor on duty wants to refer my dad for the scan that I will be paying for and my mum has been asked to take him home and start physiotherapy, so just wanted to ask what you think about that and if possible you can refer him or whatever you can do from your end will be much appreciated.
Thank you

I am in the USA and can't refer a patient in Africa for scans.
