Hello and thank you for your question.

healthydoc :

When did you have MRI?

healthydoc :

Customer:

thank you.. i had my

Customer:

last MRI last year

healthydoc :

You're welcome. Since when you are having this pain?

Customer:

EMG done done before i had my sx showed only mild delay c6,7,T1

healthydoc :

was this also done last year?

Customer:

the pain comes and goes

Customer:

EMG no done in 2011

healthydoc :

Okay. As you already had delay in C6, C7 I would suggest to repeat the EMG and MRI as well. Your symptoms does suggest that you can have cervical radiculopathy.

Customer:

i get variable pains ... involving mainly the index and thumb

Customer:

sometimes the whole had feels numb

Customer:

carpal tunnel syndrome excluded

healthydoc :

It is also possible that you have arthritis which can also cause inflammation leading to nerve irritation.

Customer:

as i siad MRI review by number of radiologist and spinal surgeon .. everybody agreed the surgery was ok

Customer:

recently seen a surgeon who thought i might be suffering thoracic outlet syndrome

healthydoc :

Do you have neck and shoulder pain, any discoloration to extremities?

Customer:

i have shoulder and neck pain ad spasm but no vascular elemants

healthydoc :

okay. So you may have this condition which can also cause pressure on the nerves leading to such symptoms. However you would also need MRI to confirm the diagnosis.

Customer:

sleeping seems to take the pain away as i am ok when i wake up in the morning

Customer:

MRI of the mck or the solder?

Customer:

neck or shoulder?

healthydoc :

Both neck and shoulder

healthydoc :

Your doctor may also order MRI of the chest.

Customer:

x ray done showed no cervical rib

healthydoc :

I would suggest to get MRI of your neck again as it has been a year.

Customer:

in fact i had more than one MRI .. last was year ago... all show the nerve root to be free... do think there is chance that the root were injured during my surgery

Customer:

?

healthydoc :

if it was injured than it would show in MRI.

Customer:

ok

healthydoc :

I would suggest to repeat MRI and EMG.

Customer:

ok thank you

healthydoc :

Physical therapy and medications like lyrica can help to ease the pain.

healthydoc :

You're welcome

Customer:

i took lyrics for some time with no real benefit

Customer:

the only thing that help is massage

healthydoc :

okay. You can try gabapentin.

Customer:

if it was still root problem can you suggest any nerve block?

healthydoc :

First you need to get evaluated if you have any nerve compression. If you have than you can get epidural or corticosteroid shot.

Customer:

gabapentin .. what dose ? i am 80 kg

healthydoc :

Start with 300mg on day 1 and than 600mg on day 2 and thereon.

healthydoc :

Always ask your doctor before taking any medications.

Customer:

any other investigation i should do?

healthydoc :

No. Just get EMG and MRI done again.

Customer:

thank you

healthydoc :

You're welcome.

healthydoc :
