My 2 year old son always is constantly constipated ever since he clocked 18 months. We have tried taking lots of water but his poops always comes hard and difficult.

For good bowel movements give him prune/apple juice. You can also give miralax which is a safe laxative.

The above mentioned measures would take care of his bowel issues.
But since he has been having this issue for quite sometime, tests are to be done to find the cause.
The most common cause is functional constipation which most often results due to poor toilet training. The other common causes include a poorly functioning thyroid or a disease called Hirschsprung Disease (HD).
Thyroid issues can be diagnosed with a blood test and HD can be diagnosed with barium xrays. I advise you to get these two tests done.

A clinical examination however is the first step. The doctor would do a rectal examination and see whether the rectum is free or loaded with hard feces. If it is loaded with hard feces, it is an indication that your son might be having functional constipation. If it is empty, it may point towards Hirschsprung disease.

Also try to avoid medications like cough syrup which will worsen constipation.

So the things to be done are -

1. Give prune/apple juice
2. Give miralax
3. Consult a pediatric gastroenterologist for a clinical examination including a rectal exam
4. Get him tested for hypothyroidism and hirschprung disease.
