If you are transferring to the other person for no payment then all you need to do is sign a transfer deed and register the transfer with the Land Registry - https://www.gov.uk/government/.../TR1__web_friendly_PDF_.pdf
The issue you have however is you still may have to pay Stamp Duty Land Tax on the approximate sale value of the property. This based on a rouch calculation would be approximately £2,700 on a property worth £150k.
