yes, my tenant has moved out and i don't know who is in there, they have also changed the locks

Ok. You need to write and give a notice to quit. You need to say unless they move out within 14 days you will go to Court. You can address it to persons unknown.
If they do not move out then you can issue proceedings for possession. You need to use form N5:

The Court will list the matter for hearing and decide whether to give possession. Once the Court does and if they refuse to move out then you can instruct bailiffs.

ok is there any special way i have to deliver this or can i just put it on the door, and also is there any special wording i need to use as I'm not too good with things like this

Just post it or hand deliver. You need to ask them to move out, they have no right to stay or say you will seek a Court order for possession.
