This problem sounds like a hardware issue (defective camera), but sometimes the same issue can be caused by glitch in the software. If there is no hardware issue on the device, some of the solutions below should resolve this issue.
Please follow the steps below to reset your device:
1. Turn off your phone
2. Remove battery and SIM card (if you have SIM card in your phone)
3. Remove SD memory card (if you have SD card in your phone)
4. Wait 2 minutes
5. Replace SIM card and battery but not SD card
6. Press and hold Power button for 30 seconds.
If the previous procedure doesn't work, perform the procedure below - factory reset (this procedure will delete data from internal phone memory, but not from your memory card and SIM card - contacts will be safe, photos and videos too):
1. Press the "Menu" soft key.
2. Press the 6 key or select Phone Settings.
3. Press the 9 key or select Reset Setting function.
4. Enter your phone's password. By default, the phone's password ***** "00000000" (eight zeros).
5. Press the "OK" soft key.

Sorry my bad, not "Phone settings", in Settings menu select number 11 - "Reset Settings" option.
Menu > Settings > Reset Settings > default code is "00000000" (eight zeros), if changed enter your password > confirm with "OK".

Correct, but do not enter "Phone Settings" just "Settings" menu.
In Settings menu select option #11 - Reset Settings.
Menu > Settings > Reset Settings > default code is "00000000" (eight zeros), if changed enter your password > confirm with "OK".

Nothing worked, so I assume it must be a hardware problem.

No need for apologies.
Unfortunately, if none of the previous solutions works there is an hardware issue (defective camera) on your phone.
Replacing the screen on your phone would be very expensive since the screen is a majority of the cost of the phone.
If you decide to repair your phone in some of official Samsung repair shop here is a phone number - 0330 SAMSUNG (726 7864).
Sorry I could not be of any more assistance, but there is a hardware issue that can't be resolved online.
