A few months off and on. I've tried heaps of different moisturiser

That looks like a mild case of rosacea--sometimes called adult acne

Nothing too serious

For now you can try cleaning with benzoyl peroxide and then applying Neosporin or another kind of antibacterial ointment twice a day

But a better plan would be to see your doctor for this and to get a prescription for a month's worth of oral doxyxyline and topical clindamycin gel

Nothing specific just like regular teenage acne
