Customer Question

I have these red scaly bumps around my nose. It started just as dryness around my nose. It started when I was in Costa Rica about a month ago. It has progressed to be further out from my nose.I used to have perioral dermatitis but it did not affect this area. It was around my mouth, my eyes, and on my forehead. About 2 years ago, I was able to get rid of it by cutting out all products with fungal feeding ingredients as well as using topical niacinimide and zinc. I had fungal acne causing ingredients in my skin routine that I introduced gradually. However, I got rid of them all again about 2 weeks ago. If it was a fungus thing again I think it would have gone away by now I think. It does look slightly similar to what I experienced before.
JA: When did the bumps first appear? Have you used any medication for this?
Customer: about a month ago and no
JA: Got it. Have you seen a dermatologist about the bumps?
Customer: No
JA: Thank you so much for your time. Finally, is there anything else in your medical history you think the Dermatologist should know?

They look like seborrheic dermatitis --a benign condition.

You can try over the counter Hydrocortisone ointment 2 times daily for 10 days over the bumps.

Customer: replied 14 days ago.
with my history of perioral dermatitis, I dont think a hydrocortisone is a good idea
Expert:  Dr Basu replied 14 days ago.

You can use moisturizer like petrolatum jelly daily.

Now there is no infection --so Hydrocortisone can be used.

Another option non steroid will be Selsun blue shampoo application over the area.

Customer: replied 14 days ago.
I have been using moisturizers for the past 3 weeks as well as anti fungal ingredients already
Expert:  Dr Basu replied 14 days ago.

Will not help --this is not an infection.

Customer: replied 14 days ago.
why would a dandruff shampoo be recommended then? what are the ingredients you are suggesting that will help
Expert:  Dr Basu replied 14 days ago.

Selenium sulfide may help

Expert:  Dr Basu replied 14 days ago.

Your best option will be steroid ointment.

You can confirm from a local dermatologist.

Customer: replied 14 days ago.
selenium sulfide is an antifungal?
Customer: replied 14 days ago.
If I have been using antifungals already and no change at all has occurred, what does that mean?
Expert:  Dr Basu replied 14 days ago.

No antifungal will help you.

This is inflammation from immune system attacking your skin layer.

There is no infection.

Customer: replied 14 days ago.
then I'm confused why the products you are telling me are anti fungal
Expert:  Dr Basu replied 14 days ago.

No it is not antifungal.

Hydrocortisone is a steroid.

Selenium sulfide is also anti inflammatory medication but weaker than steroid.

Expert:  Dr Basu replied 14 days ago.

I did not suggest any antifungal so far
