Which handset (make/model) do you have?

The most likely reasons are either a faulty charger or a worn out battery.

Does the phone work with the charger plugged in?

Does the phone work for a short while with the charger unplugged?

If the phone works for a short while when out of the cradle, or you have found that phone has required charging more often, then it's most likely that the batteries need replacing. You will need 2 Ni-MH Rechargeable AAA size batteries which are installed by sliding the battery compartment cover off of the back of the phone as shown on pg 11 of this manual:
