Extreme fatigue slow pulse

I have an underactive thyroid and I am currently on 50mg eltroxin. I have been off work for the past four weeks with extreme fatigue in legs and arms, some light headedness and difficulty concentrating for 5-6 weeks. My pulse is 45. GP Blood tests are normal. Heart rhythm and structure are ok and chest x-ray is clear. I suffer with asthma and have chronic rhinosinusitus. I feel shaky and cold all the time. I am 41 years old, should be working full-time and at present have trouble with the simplest household chores. Getting upstairs is difficult depending on how much I have exerted myself that day. I have seen my gp every week for the past 5 weeks, attended A&E, cardiologist and ENT consultant. I would appreciate any advice. Thank you
