Firstly, as a victim of domestic abuse, you will most likely qualify for legal aid to help with these applications.
In order to access legal aid, your quickest route may be to go through a charity such as women’s aid, the details are here: https://www.womensaid.org.uk/
They can help put you in touch with a lawyer locally that’s can make the legal aid application.
How old are your children please and is any contact with their dad agreed at all at the moment?
Did they witness any of the domestic abuse?
Hello again, I didn’t hear from you so I’ll give a general answer, to which you can always come back to me for clarification or further information.
As I said above, you should qualify for legal aid and the charity I sent information about will help with that.
But, if you decide to do this without a lawyer, then you will need the following forms:
Non molestation order:

Child arrangements and prohibited steps:
https://www.gov.uk/government/publications/form-c100-application-under-the-children-act-1989-for-a-child-arrangements-prohibited-steps-specific-issue-section-8-order-or-to-vary-or-discharge
For the child arrangements, you will tick the domestic abuse exception from attending mediation.
If you need an urgent hearing, deal with this in section 6 of the form.
You may also need to submit a c1a supplemental information form if the children have witnessed, been exposed to, or suffered arm as a result of them domestic violence.
