Is it a criminal offence to fill out a doctors progress report for my ongoing insurance claim even though it is a genuine claim its just on my last 2 forms I didn't go to the doctors to get them signed I did it myself using a homemade stamp with the surgery name on it. Because I was getting embarrest going every month to the surgery.

I made up a signature as I don't see any one doctor I just go and see who is available,

so no I didn't forge any particular signature ,but I did make an ink stamp with the surgery name on.



That is an offence I'm afraid. That is just simply a forged document. Even if you didn't actually forge the signature the document is clearly not genuine.

Also, its an insurance fraud I'm afraid. The original claim may have been genuine but this extension is not at least not on the basis of the evidence that you've provided.

I'm very sorry if this is bad news but I have to give you truthful information.

expensive mistake.
