We have a mobile app product which we sell on the App Store and have been approached by a TV show who want to feature the app on the show. They have asked for clearance to use it which we are more than happy with and then said "We would need permission to use it for World, All rights, in perpetuity.". We need to know what exactly this would mean for us and how we grant this even if it's just an email response or whether we need a more formal document?!

I assume they mean they want the worldwide rights to the particular programme featuring the app and not the app itself. Assuming that to be the case then you should not be bothered about this on the basis that the wider circulation of the programme the better exposure for you and the app.
As there is no restriction of the circulation (worldwide) you shouldn't need a formal document.
