Is it a crime if you have had an industrial injury every thing recorded properly then Drs notes go missing.Then be treated appallingly at all Hosp. As though you are making it up.

Loss of notes alone would not amount to an offence.
Obviously if the notes were deliberately disposed if to stop you making a claim then that can amount to an offence against public justice.
If it is just that they have been lost or, more commonly, not recorded properly then that doesn't amount to a criminal offence though.
