On January 7th I had a bad accident at work I'm a self employed

telescopic forklift driver I was working for bellway homes at Chalton hays filton got to 1 o'clock so I parked up and started walking through the back yard towards the canteen , I noticed a pile of fiberglass canopy the top one was hanging of so I thought I would make it safe and put it back on the pile , I bent down to pick up the side that was hanging of and as I lifted it I slipped on mud and rotting cardboard that has been there for a long time I caught my arm twisting it badly , I was led on my stomach with a badly dislocated Shoulder and calling for help as all the trades was on lunch break avenchaley the finishing for an came down from the office saw me led there and called for a amblance then called for help to get me up out of the mud and rotting cardboard , I was absolutely coverd, the ambulance came and gave me morthean and gas and air before putting me in the back of the ambulance and cutting my clothes of then taking me to southmead hospital , I had more morthean and lots of gas and air they put my shoulder back in its socket that was very very painfull they have found out since I have parshley severd nerves I have limited use of right hand which could be permanently damaged. My ( solicitors at the moment ) are silverbeck rymer are saying that I need a witness to me slipping which there wasn't because they were all on lunch break, two days after the accident my brother and a friend went to the site drove me to the site to pick up my car I looked at the place we're I had my accident and it had all been cleaned up ..

You do not need a witness per se who saw your accident, but this does of course help to support your version of events. However, you could ask those who first arrived at the scene and attended upon you for a witness statement.

Nevertheless, this accident should have been reported and recorded in the site accident book.
