My Child is sick I have an order stating that the Mother

Hi, My Child is sick I have an order stating that the Mother has to keep me informed of anything medical, educational or religious.
My Child was sick on Wednesday, I wasn't informed until I reached school to pick her up for my contact and then only a text message saying she wasn't in school - by which time it was already obvious she wasn't there.
The School informed me that she had be throwing up in the morning and had been sent home. I picked my second child up from the school and she informed me that my 4 year old daughter had been throwing up in the night but had still been sent to school.
The Mother has refused to tell me what is wrong with Catherine, what has happened to her , what medication she has been given or whether she has been seen by a doctor. The only message I've had back is one line'she hasn't been sick again' yesterday evening - which contradicts that on Wednesday she denied Catherine had been sick at all .
The Mother refused to let me see my daughter at contact when I dropped my other daughter off saying she was 'fine' but she hasn't been in school since.
This isn't the first time this has happened and I have received no information regarding the Children's health for the past 4 years - including when there has been a need for ambulances to be called to the mother's home.
I think it is time that I tried to get the order enforced as to be honest I have no indication of how badly ill my daughter is and I have no proof that she is even still breathing.
I'd like to know what I can do and help sending a letter stating clearly what information she needs to provide quoting the relevant laws if necessary.
