Sensitive NHS documents being sent to a previous address (twice removed) and a complete whitewash of a so-called 'investigation with PALS. I wish to know if it's possible that tampering of electronic records can leave a trace

I have every reason to believe that incriminating (for the NHS) records have been deleted from my medical records. Am I legally obliged to request proof of tampering? Is an electronic 'footprint' left behind if tampering (i.e. deletion of notes) has indeed taken place?
