Customer: replied 4 months ago.
No. as I said above.
there are 4 boxes at the start of the application asking
1 - by application pending proceedings y/n
2 - under statutory provision _____
3 - the application is made under part 8 of Civil Procedure Rules y/n
4 - this application raises issues under the human right act y/n
I am asking on whether these boxes should be ticked / filled

Pending proceedings.

That box only.

Can I clarify anything for you about this today please?

Customer: replied 4 months ago.
in another section of the form it asks to specify what the order is for
I have out to forbid the defendant from
dissapating assets including but not limited to the following addresses
and listed the properties he is known to.own
and then followed this with the term that
he must provide a disclosure of all assets up to the sum of the claim of 150000 and proof of reasonable living expenses ( for the purpose of freezing bank accounts)
does this sound reasonable?

Yes, that seems fine.

Does that assist?
