Particulars of claim can also go in the claim form - but for more complex cases particulars of claim are attached to the claim form to set out the basis for your claim - so your allegations against the defendant. Especially if there is more than one defendant - it needs to comply with CPR rule 16.4 which you can see here:

a schedule of costs listing why and what I ask
