It appears that your employer is offering to terminate your contract of employment and compromise any and all possible claims under the employment rights legislation by way of a mutual Settlement Agreement signed by both parties.
The law governing Settlement Agreements requires them to be concluded either through ACAS by way of their COT3 Settlement Agreement format, or if they are in the employer’s draft, the employee must receive independent legal advice on the form and content of the Settlement Agreement by a solicitor on the Roll of Solicitors and practising under the umbrella of a law firm fully authorised and regulated by the Solicitors Regulation Authority and covered by a Professional Indemnity Insurance policy. The employee’s solicitor must attest a schedule of the Settlement Agreement verifying that the employee has understood its contents before they signed it.
