If it's divorce proceedings - you need to apply for divorce at your local family court. The court fee is £550. If you file for divorce you will be the Petitioner and she will be the Respondent. In terms of contact, you both have equal rights to see the children and be part of their upbringing.
You would need to seek mediation first with a mediator and if you agree contact arrangements that no need to go to court. If you do not agree or she does not attend = you will need to make application using form C100 - Child Arrangements Order. The court would then decide with the use of CAFCASS who will decide what is best for both parties and the children.
Any financial claims/orders sought would need to be put down on the divorce petition.
