I am very sorry to read of the above. For the avoidance of doubt, presumably I would be correct to presume that if the cruise company has altered the date of the original cruise you booked, they have offered a refund as an alternative to booking a new cruise. Is that correct please?

Notwithstanding the above, in general terms if the cruise company cancels your existing cruise or attempts to substantively change the dates of the cruise, this would amount to a breach of contract on their part and they would be liable to offer you a refund.
as an alternative, they may offer an alternative cruise which it would be entirely within your discretion whether to accept or reject in favour of a refund. If you were minded to accept an alternative cruise, the cruise line in those circumstances would be entitled to impose such conditions as it reasonably sees fit including a minimum spend threshold on that replacement cruise save of course for the proviso that it is always open to you to demand a refund instead
