Hello there! My name is***** and I will be assisting you today. Can you switch users and see if you have the issue system wide? If you dont have another use follow the steps below.
CLick apple icon
select system preferences
select users (or accounts in earlier versions)
then Click the plus icon ( underneath the list of users)
and select to make a admin user
you can use whatever name you like as long as it is not the same as your current user
then click ok
select the apple icon and select sign out
and sign in to the new account
is it the same?

Dear *****ny,

although I am the only user and manager of the system, it appears that I have no record of my password! This is not your fault and I have noted your reply. However, I cannot follow up your instructions because I cannot add a new user name.

no worries we can change the password ***** you forgot it. Just tell me what osx version you have.
TO find this out

Good. My OS X version is 10.9.4

When you forgot the password ***** you can't use OS X, you have to use the new Recovery.
To boot in Recovery system, press Command and R keys in boot and hold the keys until you see the Apple icon. f your Mac has got a wireless keyboard, hold them when you hear the startup sound
When it starts, select Utilities > Terminal, and type:
Press your user and type your password. Finally, reboot.

Sorry to be dim - I am 82 but started to compute in 1952 on EDSAC1 at Cambridge. Have pressed cmd and r with no result. Must it be R and not r and where is 'boot'.?

I am using OS X to communicate with you. i am the only user and have never logged out!

No worries, What i meant was hold those keys while your mac is off

then turn on your mac continuing to hold those keys then the reset of the steps will make sense.:)
