It will help if you could provide some further information.
Is it the nipple that is larger than the opposite side?
Or is the breast larger than the opposite side?
Can you feel any lumps in the breast tissue deep to the nipple?

Itching of the nipple is most commonly caused by a fungal infection.
This can be treated with an antifungal cream like Clotrimazole.
However, in some cases, there can be a type of breast cancer called Paget's disease of the nipple that can present like this.
So this definitely and to be evaluated by your doctor and referred to the breast specialist.
Meanwhile you can try a over the counter anti fungal cream like Clotrimazole or Miconazole.

I have answered this twice but the answer is my breast is larger than the other not the nipple, I may nave sent it wrongly but thank you doctor

Larger breast can be due to the excessive breast tissue --that can be normal at your age.
However, there can also be lump or cancerous changes in small cases.
That is why you will need further evaluation with mammogram, breast ultrasound.
