I have shingles, am 76 and went to my doctor the day the first rash appeared having had pain and sensitivity in the affected area. He did not think it was shingles, though the warning signs were quite clear. He also said that if he was wrong and it was shingles, it was already too late for antivirals.
The rash was much larger the following day, Friday, but I did not return to the surgery believing it was pointless. However, in the meantime I discussed it with others who had had shingles, and also researched online.
After the weekend I returned to the doctor who confirmed it was shingles, but reiterated that it had always been too late for antivirals, I asked what indicated the onset, symptoms or rash and he said the preceding pain and sensitivity. Therefore it was already 3 or 4 days in by the time The rash appeared.
After an awful night I did some more looking online concerning this specific point, and discovered that in fact the timing started with the onset of the rash. Treatment, especially for older people would be beneficial, particularly in reducing the possibility of recurring pain after the initial attack. This should start within 3 days, but would also even be helpful within a week. My rash appeared as one spot a week ago today. I am intending to ring my doctor to ask for retroviral treatment. I do not want to be confrontational but I think it important for me to get the best treatment. What do you think?

I think that a course of antiviral treatment is required in your case and I would have used antiviral medications.

For the pain, if it is mild you can use over-the-counter pain medications like ibuprofen and if severe neuropathic pain medications like pregabalin, duloxetine, gabapentin may be used.
