That is a great question sir.
The studies seem to show that after age 70, treating prostate cancer with removal of the gland probably does more harm
There is no way to know if this is cancer or just enlargement of the gland without a biopsy
I would do the following
1)retest in a few weeks and see if it is really high
2)if it is then you have to make a decision. Do you want to pursue ruling out cancer or just leave it alone
If it were me and I was otherwise VERY healthy with no other issues, I would treat it.

What does the score of 6.5 mean

By itself? Not much.
It could be elevated from benign enlargement of the prostate, infection or prostate cancer
There are many possible causes.
We don't really know until you have a biopsy
