I get a strange sensation wen passing urine it starts at the nape of my neck over my head goes down my arms to the tips of my fingers but not all the time

when you pass urine, you use both of your parasympathetic nervous system and sympathetic nervous system

the parasympathetic nervous system helps you relax your pelvic floor muscles and the sympathetic nervous system tells your bladder muscles to contract and push out the urine.

the parasympathetic nervous system can cause some tingling and sometimes even twitching of your neck and arm muscles as you try to relax to pass urine out.
