you could have a lung bronchitis infection or pneumonia.
you need to go back to your doctor and get a chest xray and sputum culture of phlegm which you are coughing up
you most likely need a course of antibiotics to help kill bacteria which is most likely causing your fever and cough symptoms.
39.5c is 103F which is a high fever.
I'm surprised your doctors have not started you on antibiotics so far
