painful swelling in the back of the ankle could be due to tendonitis or a blood clot or cellulitis or an infection or bad arthritis or gout possibly.
you should go to your GP or to the local hospital and get your leg evaluated
you might need a dopler ultrasound of the area to check if there is fluid in the area or if there is a blood clot in the ankle or an infection.
blood test can check for elevated WBC white blood cell count which can be a sign of infection.
treatment would depend on what is found.
