It's possible that the nasal spray isn't a strong enough medication or is just getting to the lower sinuses while the trouble is elsewhere. An over the counter remedy to try to treat the symptoms more fully is an antihistamine diphenhydramine. This medication is sedating so should be tried at bedtime at about 25-50 mg dose. If this helps, it can be used for a few days to see if the symptoms are viral and/or resolve on their own.
