Why have I pain in both my shoulders, from the wrist up to my neck, especially when I reach outwards for anything?

Most likely: disc problems in the neck.

This requires evaluation, and probably an MRI of the neck.

physical therapy and medication is the first line of treatment.

you don't need an MRI

In fact, most socieities don't recommend this until you fail conservative measures

go to your general doctor and get an exam. Xrays of the spine can tell a lot

Also get tested for autoimmune disease like polymyalgia rheumatica. These are common in your age group
