I have suffer from insomnia for the last 4 years. I feel mentally exhausted and was told that I have dissociation amnesy . Can you help with the insomnia!
JA: Have you seen a doctor about this yet? What medications are you taking?
Customer: I am under the treatment of a psychiatrist at the moment but not very happy with the treatment received. I take fluoxetinr 40 mg, amitriptyline 100 mg, atorvastatin 10 mg, aripripazole 10 mg
JA: What medications do you take daily? Are you allergic to any medications?
Customer: I also take odefessy, an antiretroviral. I take all these medications daily
JA: Anything else in your medical history you think the Doctor should know?
Customer: History of depression, PTSD, bipolar

You should try mirtazapine 7.5 to 15 mg at bedtime if not tried before instead of amitriptyline.

Mirtazapine dose can be increased to 30 mg if needed.

Did you have a sleep study done to check for sleep apnea?

I have had a sleeping study which was negative vfor apnea or any respiratory disturbance. I have also taken mirtazepine, and I am now on 100mg if amitriptyline.but still only getv2 to 3 hours sleep per night

Ok in that case, you will need to be tried on a different regimen given that you are having problem maintaining your sleep.

Zolpidem 5 to 10 at bedtime should be the next option.
