Do this for 2 weeks then stop the evening dose forever
make sense so far?
after these 2 weeks, when you stopped evening dose forever, then do this:

No I don't think that's related if you've been using it that long.
I don't think so. It's worth asking your GP when you see them to listen to your heart and check your blood pressure, pulse and they should check your ECG as well and this will help them find out.
It is possible, I suppose you will know better once you reduce the dose and see if things get better
