Based upon the information I would ask to get an HBAIC levels plz. It gives you the average sugar Levels over the past three months. As you haven't been checking your sugars regularly so this is important to establish the control.

Get all your bloods done especially the renal function to see if there is any effect on the kidney.

Get an ecg and echo too plz.

And lastly do see an ophthalmologist. The effects of diabetes on the eyes need to be seen
