Looks like the balance shaft has sized , remove the Radiator and pull
out the shaft and check the shaft for damage ,bearings Damage ect .

Have a engine in the shop being repaired ,i notice there was no balance shaft belt fitted at all ,called the customer and he said he removed it 3 years ago ?? 45.000 km driven with out any problems , but not saying this is good as you will start to have engine vibration on low rpm .

But yes you can run without the balance shaft , oil pump is run off the crankshaft and not the balance shaft belt like many think.
